Workdays/Hours:                  Fulltime: Monday-Friday 8 a.m. to 5 p.m. (Some shared weekend hours may be required.)

Position Summary:

The Total Joint Patient Navigator/Case Manager will provide optimal quality and continuity of care from the pre-operative setting through one year post surgery for the Ambulatory Surgery Center patients and for the Comprehensive Joint Replacement program. This includes patient education, periodic surveys, data abstraction and analytics.  They will act as a resource for patients and healthcare partners and work collaboratively as part of a multidisciplinary team.


  • Current unrestricted Registered Nurse license in the state of Tennessee required
  • Bachelor’s degree preferred
  • Minimum of 3 years Registered Nursing experience required
  • Orthopedic experience preferred
  • Case Management experience preferred
  • Excellent interpersonal and customer service skills

  • Ability to provide telephonic Patient Navigation as well and direct patient interaction.


Essential Duties and Responsibilities:

  • Responsible for managing the ambulatory Total Joint Replacement (TJR) patients and Comprehensive Joint Replacement Medicare programs through the pre-operative phase for one year. This includes telephonic and direct Patient Navigation utilizing current software.
  • Obtain patient surveys/questionnaires throughout the year for submission to the American Academy of Orthopedic Surgeons (AAOS) Registries.
  • Critically review, evaluate, and analyze medical records for required data for the AAOS Registry to include outpatient and inpatient TJRs.
  • Provide analytic reporting to management as requested.
  • Coordinate with all participants in care, i.e., patients, surgeon team, caregivers, outside Case Managers and Social Workers, Therapy, ambulatory surgery team.
  • Provide education to patients and healthcare partners in face-to-face class and Zoom meetings.
  • Maintain expertise and participate in processes to ensure data accuracy and reliability.
  • Deliver all appropriate measures accurately and by scheduled deadlines.
  • Other duties as assigned such as providing Case Management in other OrthoSouth and hospital-based programs.
  • Assist in Covid-related issues as needed.
  • Report TJR complications as indicated.
  • Be available by cell phone to patients and management, as needed, which may be after hours.
  • Provide excellent customer service to internal and external customers to include patients and families.
  • Assist on continued improvement of department/company programs.
